The Beyer Family Senior Case Management program is a program for Jewish Seniors and their families. Seniors and families come to us in a variety of ways; the Senior referring themselves, the family referring or a Rabbi or social service agency making the referral. This is a voluntary service for Seniors. Fees are on a sliding scale. No one is turned away.
Services in this program include an assessment and development of a Case Management Plan, implementation of that plan and when needed referrals and linkage to other agencies. The majority of services are done in the field at the client’s home. Many families who have Senior parents in the area contact us to provide assessment and service when they live out of the area.
The Senior Case Manager, Kathleen Oates LCSW,LCS is an experienced social worker with many years of experience working with Seniors. At times, MSW students may work with our Case Manager and assist in providing services.
Donations to this program go directly to the Seniors and can make a big difference in the life of a Senior.
